When cancer is suspected, tissues from biopsy specimens are examined under the microscope and, based on the characteristics of the individual cells and the patterns they form, a specific diagnosis is made. This process has not changed fundamentally for decades, even though it is widely accepted that biological and molecular properties of tumor cells may be much more predictive of the tumor's aggressiveness and the patient's response to treatment. Often only a fraction of cancer patients respond to given cancer therapy, so research has long sought to identify biomarkers that could predict response to therapy and thus facilitate selection of the most appropriate therapy for a given patient. But despite considerable effort devoted to finding diagnostic markers, progress has been slow. Only a handful of markers, such as the estrogen receptor status in breast tumors, are routinely used to guide treatment decisions.

Recent technological advances in biomedical research, especially in genomics and proteomics, have made it easier to identify many new markers of cancer that could potentially improve cancer screening, diagnosis, and treatment. But there are still many obstacles to developing clinically useful biomarker tests, including technical challenges associated with validating potential cancer markers, and challenges associated with developing, evaluating, and incorporating the screening and diagnostic tests that make use of those markers into clinical practice. However, successful development of cost effective tests could have a profound impact on the way cancer is detected, diagnosed, and treated, and thus could reduce the economic burden of cancer.

The goal of this workshop is to identify and discuss key policy questions regarding the development and use of biomarker-based tools. The workshop will include small group discussions during which participants will have the opportunity to propose ways for overcoming obstacles to progress in this field. Topics will include 1) the potential to improve cancer screening, diagnosis, and therapy through the use of emerging biomarker technologies; 2) current limitations of genomics and proteomics technologies for cancer detection, diagnosis, and drug development, and steps that could be taken to improve them; 3) the logistics and cost of coordinating the development of biomarkers and targeted therapies; 4) regulatory oversight of biomarker development and use; 5) adoption of biomarker-based tests and therapeutics into clinical practice; and 6) some of the potential economic implications of adopting these emerging technologies.
